6th Semester Placement Forms & Information

Students that are in the third year of the CSI program are required to complete a placement with a minimum of 350 hours from January to April 2022. We ask interested organizations to provide a job description that we make available to all eligible students.

Prior to the placement starting, there are 2 forms for the employer to fill out and 2 forms for the student:

  1. Fleming Employer Acknowledgement & Compliance Certification – Students will have to follow the Field Placement sites’ specific COVID-19 protocols.  This will only change at the directive of the COVID Protocol Committee, based-on input from provincial and municipal health units.

  2. WSIB FormLetter to Placement Employers Process for Workplace Safety and Insurance Board Coverage.

Student Forms

  1. Freedom of Information –   It is program policy that if the faculty has information concerning a student that is relevant to, and may affect future performance in field placement, it is important that the information be shared with the agency supervisor. 

This waiver authorizes Fleming College to collect information concerning the student’s field placement experience from their field placement opportunity.  The waiver also authorizes the field placement employer to release information concerning students’ field placement experience to Fleming College.  Finally, it authorizes field placement faculty to provide relevant information to the field placement employer in the form of a reference.

  1. Fleming COVID19 Placement Informed Consent Form – Students will have to follow the Field Placement sites’ specific COVID-19 protocols.  This will only change at the directive of the COVID Protocol Committee, based-on input from provincial and municipal health units.

Throughout the 4-month placement, employers are to provide a mid-term report and final report of the student’s progress from their experience within the organization.  A template of this report is provided to you.

Course Selection & GNED Guide

Course selection can be a daunting task, especially since the onus is on you, the student, to ensure you have taken the necessary courses to graduate. This guide aims to explain and simplify the course requirements and selection tools at your disposal.

General education (GNED) requirements can be confusing, so here are the essential details. For each year in your college program, you are required to take a GNED. Example: The CSI program is three years long, therefore you must take three GNEDs before you graduate. Most GNEDs are optional, but occasionally a program will include a mandatory GNED. Mandatory GNEDs are included in the program outline and do not need to be registered separately. Example: Working with Business & Industry (ORGB 21) is a mandatory GNED within the CSI program.

For the CSI program, students must complete three GNED courses to graduate. Out of these three GNED courses, students only need to register for two separately since the mandatory GNED, ORGB 21, is included in the program outline. Lastly, GNED requirements are determined by the Ontario Ministry of Colleges, not Fleming College.

How do I know what coures I need to take?

The Academic Requirements PDF displays a broad overview of your course requirements and status. To view this PDF, follow the instructions below:

  1. Login to your MyCampus dashboard and navigate to the Student Centre.

  1. From the drop-down menu on the left-hand side of your schedule, select Academic Requirements. Press the arrow icon on the right-hand side of the menu to visit that page.

  1. At the top of the Academic Requirements page, select View Report as PDF to view the page in a simple PDF format.

  1. Carefully read through the PDF and identify any descrepancies.

I’m missing non-GNED courses, what do I do?

The Timetable Tutor guide is the simplest means of adding non-GNED courses to your timetable. To add non-GNED courses to your timetable, follow the instructions below:

  1. Navigate back to your Student Centre, then select the Timetable Tutor link from the right-hand sidebar.

  1. From the Timetable Tutor’s navigation menu, select Enrollment Add.

  1. From the Enrollment Add page, select the link titled PT Registration form for Upper Semester Courses.

  1. Follow the instructions on the form to add the missing non-GNED course, then submit once finished.

I need to register for a GNED course, what do I do?

The previously mentioned, Timetable Tutor, has an excellent guide on GNED registration. To reach this guide, navigate back to the Timetable Tutor, then select the Selecting a General/Program Elective. Afterward, follow the instructions provided in the guide.
